How to File a Claim
- Life Claim Form - Please send the completed documents to UNUM when submitting a life claim.
- Short Term Disability Claim Form - Please send the completed documents to UNUM when submitting a disability claim.
- Long Term Disability Claim Form - Please send the completed documents to UNUM when submitting a disability claim.
- Voluntary Accidental Death & Dismemberment Claim Form - Please send the completed documents to New York Life when submitting a claim.
