• Short-Term & Long-Term Disability: To follow-up on a claim, please call UNUM at 1‑800-858-6843 to speak with a representative.
• Dental: To obtain information regarding providers in your area, review plan benefit details, or to follow-up on a claim, please:

Forms Center

• Employee Insurance Enrollment Form - Complete this form to enroll in any of the insurance programs offered by your employer or to update coverage selections and dependent information.  
• Employee Insurance Change Form - Complete this form for name, address, and beneficiary changes.
• Life Insurance Election of Portability & Current Portability Rates Table - You may be eligible to continue your Life and AD&D coverage.  To apply, complete this form and send it to UNUM within 31 days after your group coverage ends. 
  • Life Insurance Conversion Application - for those who may not qualify for portability coverage, but wish to continue life insurance.

How to File a Claim

• Short Term Disability Claim Form - Please send the completed documents to UNUM when submitting a disability claim.
• Long Term Disability Claim Form - Please send the completed documents to UNUM when submitting a disability claim.
• Dental Claim Form - please provide the CIGNA Group/Plan ID # 3217108 to you dental provider for electronic processing of claims. If your dental provider does not file electronically, you may file with the form.
• Vision Claims - Most eye care providers will file electronically. Online filing of claims is available at www.vsp.com if your provider does not file electronically.
• Voluntary Accidental Death & Dismemberment Claim Form - Please send the completed documents to New York Life when submitting a claim.  You may also submit your claim online at www.newyorklife.com/claims or by calling (800) 362.4462.
